A Spotlight On Convenient testosterone therapy Systems

A Harvard Specialist shares his thoughts on testosterone-replacement Treatment

It could be said that testosterone is the thing that makes guys, guys. It gives them their characteristic deep voices, big muscles, and facial and body hair, distinguishing them from women. It stimulates the growth of the genitals , plays a role in sperm production, fuels libido, and leads to regular erections. It also fosters the production of red blood cells, boosts mood, and aids cognition.

Over time, the testicular"machinery" that makes testosterone gradually becomes less effective, and testosterone levels start to fall, by about 1% a year, beginning in the 40s. As guys get in their 50s, 60s, and beyond, they might begin to have symptoms and signs of low testosterone like reduced libido and sense of energy, erectile dysfunction, diminished energy, decreased muscle mass and bone density, and nausea. Taken together, these symptoms and signs are often referred to as hypogonadism ("hypo" significance low functioning and"gonadism" referring to the testicles). Yet it is an underdiagnosed problem, with only about 5% of those affected receiving treatment.

Various studies have revealed that testosterone-replacement therapy can provide a wide range of benefits for men with hypogonadism, including improved libido, mood, cognition, muscle mass, bone density, and red blood cell production. Much of the current debate focuses on the long-held belief that testosterone may stimulate prostate cancer.

He's developed specific experience in treating low testosterone levels. In this interview, Dr. Morgentaler shares his views on current controversies, the treatment strategies he utilizes his patients, and he believes specialists should rethink the potential link between testosterone-replacement therapy and prostate cancer.

Symptoms and diagnosis

What signs and symptoms of low testosterone prompt that the typical man to see a physician?

As a urologist, I have a tendency to see men since they have sexual complaints. The main hallmark of low testosterone is reduced sexual libido or desire, but another can be erectile dysfunction, and any man who complains of erectile dysfunction must get his testosterone level checked. Men may experience other symptoms, like more trouble achieving an orgasm, less-intense climaxes, a lesser quantity of fluid from ejaculation, and a feeling of numbness in the manhood when they see or experience something which would normally be arousing.

The more of the symptoms you will find, the more likely it is that a man has low testosterone. Many physicians often discount these"soft symptoms" as a normal part of aging, however, they are often treatable and reversible by normalizing testosterone levels.

Are not those the same symptoms that men have when they are treated for benign prostatic hyperplasia, or BPH?

Not exactly. There are quite a few medications that may reduce libido, including the BPH medication finasteride (Proscar) and dutasteride (Avodart). Those drugs may also decrease the quantity of the ejaculatory fluid, no question. However a decrease in orgasm intensity normally does not go together with treatment for BPH. Erectile dysfunction does not usually go together with it either, though certainly if a person has less sex drive or less attention, it's more of a struggle to get a fantastic erection.

How can you determine if or not a man is a candidate for testosterone-replacement therapy?

There are just two ways that we determine whether someone has low testosterone. One is a blood test and the other one is by characteristic signs and symptoms, and the correlation between those two approaches is far from perfect. Normally men with the lowest testosterone have the most symptoms and men with maximum testosterone have the least. However, there are a number of men who have reduced levels of testosterone in their blood and have no symptoms.

Looking at the biochemical numbers, The Endocrine Society* considers low testosterone to be a entire testosterone level of less than 300 ng/dl, and I believe that's a sensible guide. But no one really agrees on a number. It is not like diabetes, in which if your fasting sugar is above a certain level, they will say,"Okay, you've got it." With testosterone, that break point is not quite as apparent.

*Note: The Endocrine Society publishes clinical practice guidelines with recommendations for who should and should not receive testosterone therapy.

Is total testosterone the ideal thing to be measuring? Or should we be measuring something different?

This is another area of confusion and great discussion, but I don't think that it's as confusing as it appears to be in the literature. When most physicians learned about testosterone in medical school, they heard about total testosterone, or all the testosterone in the human body. However, about half of the testosterone that is circulating in the blood is not readily available to cells.

The biologically available portion of overall testosterone is called free testosterone, and it's readily available to cells. Though it's just a little fraction of the overall, the free testosterone level is a fairly good indicator of reduced testosterone. It is not perfect, but the correlation is greater than with testosterone.

Do time of day, diet, or other factors affect testosterone levels?

For many years, the recommendation was to receive a testosterone value early in the morning since levels begin to fall after 10 or even 11 a.m.. But the information behind this recommendation were attracted to healthy young men. Two recent studies demonstrated little change in blood glucose levels in men 40 and mature within the course of this day. One reported no change in typical testosterone until after 2 p.m. Between 2 and 6 p.m., it went down by 13%, a modest amount, and probably not enough to influence diagnosis. Most guidelines still say it's important to do the test in the morning, however for men 40 and over, it likely does not matter much, provided that they obtain their blood drawn before 6 or 5 p.m.

There are a number of rather interesting findings about dietary supplements. By way of instance, it seems that those that have a diet low in protein have lower testosterone levels than males who eat more protein. But diet has not been studied thoroughly enough to make any clear recommendations.

In the following guide, testosterone-replacement treatment refers to the treatment of hypogonadism with adrenal gland -- testosterone that's produced outside the body. Depending on the formula, therapy can lead to skin irritation, breast tenderness and enlargement, sleep apnea, acne, decreased sperm count, increased red blood cell count, along with other side effects.

Preliminary studies have shown that clomiphene citrate (Clomid), a drug generally prescribed to stimulate ovulation in women struggling with infertility, can boost the creation of natural testosterone, also termed endogenous testosterone, in men. Within four to six months, all of the guys had increased levels of testosterone; none reported any side effects throughout the entire year they were followed.

Because clomiphene citrate isn't accepted by the FDA for use in men, little information exists regarding the long-term ramifications of taking it (such as the risk of developing prostate cancer) or whether it's more capable of boosting testosterone than exogenous formulas. But unlike exogenous testosterone, clomiphene citrate preserves -- and potentially enhances -- sperm production. This makes medication like clomiphene citrate one of just a few choices for men with low testosterone who wish to father children.

    Formulations

What forms of testosterone-replacement therapy can be found? *

The oldest form is the injection, which we still use since it's cheap and since we reliably become good testosterone levels in almost everybody. The drawback is that a man should come in every few weeks to find a shot. A roller-coaster effect can also happen as blood testosterone levels peak and return to baseline.

Topical therapies help preserve a more uniform level of blood glucose. The first kind of topical therapy has been a patch, but it has a very large rate of skin irritation. In one study, as many as 40 percent of men who used the patch developed a red area in their skin. That limits its usage.

The most commonly used testosterone preparation from the United States -- and also the one I start almost everyone off -- is a topical gel. There are just two brands: AndroGel and Testim. According to my experience, it tends to be consumed to good degrees in about 80% to 85% of men, but leaves a significant number who don't absorb sufficient for this to have a positive impact. [For details on various formulations, see table ]

Are there any downsides to using dyes? How long does it require them to get the job done?

Men who start using the implants need to return in to have their testosterone levels measured again to make certain they're absorbing the right quantity. Our goal is the mid to upper assortment of normal, which generally means around 500 to 600 ng/dl. The concentration of testosterone in blood actually goes up quite fast, within a few doses. I normally measure it after 2 weeks, though symptoms may not alter for a month or two.

  • symptoms of low testosterone.

Therapy Isn't recommended for men who have

  • Prostate or breast cancer
  • a nodule on the prostate which can be felt during a DRE

Do time daily, diet, or other factors affect testosterone levels?

For years, the recommendation has been to get a testosterone value early in the morning because levels begin to fall after 10 or 11 a.m.. However, the information behind this recommendation were attracted to healthy young men. Two recent studies showed little change in blood testosterone levels in men 40 and older within the course of this day. One reported no change in average testosterone until after 2 p.m. Between 2 and 6 p.m., it went down by 13%, a modest sum, and probably not enough to affect identification. Most guidelines nevertheless say it's important to perform the evaluation in the morning, but for men 40 and over, it probably doesn't matter much, provided that they get their blood drawn before 6 or 5 p.m.

There are some rather interesting findings about diet. For instance, it seems that individuals that have a diet low in protein have lower testosterone levels than men who eat more protein. But diet has not been studied thoroughly enough to make any recommendations that are clear.

Within the following guide, testosterone-replacement therapy refers to the treatment of hypogonadism with exogenous testosterone -- testosterone that's produced outside the body. Based upon the formula, treatment can cause skin irritation, breast enlargement and tenderness, sleep apnea, acne, reduced sperm count, increased red blood cell count, and other side effects.

Preliminary research has shown that clomiphene citrate (Clomid), a drug generally prescribed to stimulate ovulation in women struggling with infertility, can boost the production of natural testosterone, also known as nitric oxide, in men. In a recent prospective study, 36 hypogonadal men took a daily dose of clomiphene citrate for three or more months. Within four to six months, all of the men had increased levels of testosterone; none reported some side effects throughout the year they had been followed.

Since clomiphene citrate isn't accepted by the FDA for use in men, little information exists about the long-term effects of carrying it (including the probability of developing prostate cancer) or whether it is more effective at boosting testosterone compared to exogenous formulations. But unlike adrenal gland, clomiphene citrate maintains -- and possibly enriches -- sperm production. That makes medication such as clomiphene citrate one of just a few options for men with low testosterone who wish to father children.

What forms of testosterone-replacement treatment are available? *

The oldest form is an injection, which we still use since it's inexpensive and since we reliably get fantastic testosterone levels in almost everybody. The drawback is that a person needs to come in every couple of weeks to get a shot. A roller-coaster effect can also happen as blood glucose levels peak and return to research.

Topical treatments help maintain a more uniform level of blood testosterone. The first kind of topical treatment was a patch, but it has a quite large rate of skin irritation. In one study, as many as 40 percent of men who used the patch developed a red area in their skin. That limits its usage.

The most widely used testosterone preparation in the United States -- and the one I start almost everyone off with -- is a topical gel. The gel comes from tiny tubes or in a unique dispenser, and you rub it on your shoulders or upper arms once a day. According to my experience, it tends to be absorbed to good levels in about 80% to 85 percent of guys, but that leaves a substantial number who don't absorb enough for it to have a positive effect. [For specifics on various formulations, see table below.]

Are there any drawbacks to using dyes? How much time does it require them to get the job done?

Men who begin using the implants need to return in to have their own testosterone levels measured again to be certain they are absorbing the proper quantity. Our target is the mid to upper range of normal, which generally means approximately 500 to 600 ng/dl. The concentration of testosterone in the blood actually goes up quite quickly, within several doses. I normally measure it after 2 weeks, even though symptoms may not alter for a month or two.
